Issue
After the SharePoint Online Connector is deployed to a site, you may find that the Manage Record option in the Ribbon, or in documents' context menus is missing or duplicated.

Resolution
Remove and re-deploy the SharePoint Online Connector App on the site as follows:

Unregister the App from the SharePoint Online site contents

Delete the App from the site collection

Remove the deleted App from the primary and then the secondary recycle bins

Re-deploy the App as you would for a new site
